Creating Release of Information Forms for Use by Multidisciplinary Teams: Disclosure of Health, Mental Health, Social Services, and Substance Use Disorder Information with Client Consent

Multidisciplinary teams (MDTs) bring together professionals from various fields to address complex client needs, such as coordinating care for pregnant individuals with substance use disorders, protecting vulnerable adults from abuse, and responding to cases of child abuse and neglect. However, confidentiality laws often complicate information sharing. This bulletin explains how, with client consent, MDT members can share confidential information by creating a legally compliant multiparty release of information (ROI) form. Such forms streamline the coordination of services, helping agencies work together more effectively while complying with legal requirements. By simplifying this process, MDTs can enhance collaboration, improve outcomes, and better serve the individuals and communities that rely on their support.
