Involuntary Commitment Forms

Because these forms are periodically reviewed and updated, the websites listed below should be checked for the latest versions. In addition, other forms related to the commitment process, including parts of the process not covered in this learning program, can be found at these websites.

The Administrative Office of the Courts provides the court forms that are used in the commitment process. These forms are available online at the Judicial Department website, www.nccourts.org under “Forms.” You can search for the forms using the form number, form title, or the category “Special Proceedings.”

The Department of Health and Human Services, Division of Mental Health, Developmental Disabilities, and Substance Abuse Services, publishes the examination form, supplemental emergency certificate, and other forms used in the commitment process. These forms are available online at the Department of Health and Human Services website, www.dhhs.state.nc.us/mhddsas/, under “Legal Forms for IVC” or “Legal Forms for Hospitals."
